The Total Support System From CHCH

Our Hospice program is dedicated to enhancing the quality of life, while respecting the patient and the
family's right to make decisions that affect their lives. Hospice is there to help in difficult times, with emphasis on human dignity and preservation of the quality and sanctity of life.
Hospice does this by providing a total support package system to patients and families who choose to share their last days together - at home, comfortably and with dignity.
Hospice Services
With your physician directing the plan of care, our interdisciplinary team coordinates to address pain and symptom management as well as the psychological, emotional, social and
spiritual needs of both patient and family.
Some of the services hospice provides or assists in providing are:
- Coordination with patient's physician
- Scheduled skilled nursing visits
- Personal care and respite by Hospice attendants
- 24 Hour availability
- Physical and speech therapy as needed
- Counseling for patient and family
- Volunteer support - as needed
- Bereavement support program
- Home medical equipment and pharmaceuticals as related to primary diagnosis - Applicable to Medicare and Medi-Cal beneficiaries only
Requesting Our Services
Family members, friends, health care professionals or patients may call and ask about our services at any time. We will work closely with you and your physician to
determine how we can best meet your home care needs. Services are reimbursed by Medicare, Medi-Cal and private insurance.
Patient services are provided without regard to race, color, creed, sex, handicap (mental or physical) or place of national
origan.
