Home Care
California Home Care & Hospice Inc., provides health care services in the home on a part-time basis.
Experienced nurses, therapists and aides are available to assist patients of all ages, including newborn infants, young children, adults and the elderly.
Services may include skilled observation, assessment, and instruction to our patients and families.
Our nurses and therapists perform procedures ordered by the patients physician, including I.V. therapy at home, catheter insertion, wound care, ostomy care and other skilled services to meet your needs at home. Our home health agency services are covered by Medicare, Medi-Cal and private insurance, upon order from your physician.

Hospice
Hospice is dedicated to enhancing the quality of life, while respecting the patient and family's right to make decisions that effect their lives. Hospice is there to help in difficult times, with emphasis on human dignity and preservation of the quality and sanctity of life.
Hospice does this by providing a total support system to patients and families who choose to share their last days together - at home, comfortably and with dignity.
Admission Criteria
- Cessation of Curative Treatment
- Prognosis of Six Months or Less
- Consent of the Attending Physician
Hospice Services
The interdisciplinary team coordinates an individualized plan of care to address pain and symptom management as well as the psycological, emotional, social and spiritual needs of both patient and family.
The patient's physician directs the plan of care in coordination with the hospice staff. Some of the services Hospice provides or assists in providing are:
- Coordination with Patient's Physician
- Scheduled Skilled Nursing Visiits
- Personal Care and Respite by Hospice Attendants
- 24 Hour Availablity
- Physical and Speech Therapy As Needed
- Counseling for Patient and Family
- Volunteer Support - As Needed
- Bereavement Support Program
- Home Medical Equipment and Pharmaceuticals as Related to Primary Diagnosis - Applicable to Medicare and Medi-Cal Beneficiaries Only
